LIABILITY WAIVER
In consideration for my/my child(ren)'s attendance and participation in the Martial Arts training offered by Unity Martial Arts Academy and its Instructors, I the student or parent, acknowledge the existence of certain inherent risks in the type of training and hereby agree to assume all risks. I hereby submit this registration and liability waiver form; certify that the above information is true and correct; and hereby release, discharge, and waive any and all responsibility of Unity Martial Arts Academy, UWTA, JudoKickBox, MMA Brazilian JiuJitsu, schools, instructors, coaches, medical staff and other students/competitors from liability for any injury, including death, and for damage to or loss of property which may be suffered by myself or my child(ren) arising out or, in any way resulting from or attributing in whole or in part to my/my child(ren)'s traveling to, training for, being coached in, using any sports equipment, or participating in any form of training.
